【成分】 單硝酸異山梨醇酯
【適應症】 心絞痛的長期治療的預防。心肌梗塞後的治療。與洋地黃苷和／或利尿劑配伍後用於嚴重心肌功能不全的輔助治療。肺高壓。
【用量用法】 20-40mg qd -bid
【禁忌】 低充盈壓性急性心肌梗塞，低充盈壓性左心室衰竭，休克，血壓極低，心室縮窄性心肌病，縮窄性心包炎，心包填塞。
【不良反應】 治療初期常有頭痛。在治療初期及加大劑量後可見血壓降低、眩暈及乏力。偶見噁心，嘔吐，面部潮紅及皮膚過敏反應。極少數情況下可能發生血壓大幅度下降，伴心絞痛症狀加劇。虛脫及昏厥見於個別病例。間或見剝脫性皮炎。
【注意事項】
下列疾病患者慎用：主動脈瓣和／或二尖瓣狹窄，由低血壓引起的迴圈失調趨向，與顱內壓升高有關的疾病，嚴重腎功能不全。妊娠和哺乳婦女慎用。可影響機械操作能力。應避免大劑量連續服用。
【藥物相互作用】
同時攝入抗高血壓藥、β阻斷劑、其他血管擴張劑、神經抑制劑、三環類抗抑鬱藥或酒精可加強本藥的降血壓作用。可加強二氫麥角堿的升血壓作用。非類固醇抗炎藥可降低本藥的藥效。
【規格】 片劑 20mg x 20片。其他規格還有:10mg,40mg,60mg.
Elantan - Isosorbide-5-mononitrate, the long-acting metabolite of isosorbide dinitrate, is used as a vasodilatory agent in the management of angina pectoris. By dilating the vessels, it lowers the blood pressure and reduces the left ventricular preload and afterload, therefore, leads to a reduction of myocardial oxygen requirement.
Indication: For the prevention of angina pectoris due to coronary artery disease and the treatment of acute and chronic angina pectoris, hypertension, and myocardial infarction.
Elantan is used to prevent angina (chest pain). It can also be used to treat certain types of heart failure. Elantan helps to relax and open up the blood vessels. This helps more blood and oxygen to flow to your heart. Elantan belongs to a group of medicines known as nitrates.
Elantan: Long-term treatment of coronary heart disease (circulatory disorders in the myocardium); prophylaxis of angina pectoris attacks (cardiac pain); after treatment of myocardial infarction; long-term treatment of chronically restricted function of the myocardium (chronic myocardial insufficiency), alone or in combination with cardiac glycosides and diuretics. Elantan Long: Prophylaxis of angina pectoris; long-term treatment of coronary heart disease; post-treatment of myocardial infarction with persisting anginal symptoms and long-term treatment of chronic congestive heart failure in combination with digitalis and/or diuretics. Elantan LA 25: Long-term treatment of coronary artery disease, prophylaxis of angina pectoris, post-treatment of myocardial infarction with persisting anginal symptoms, long-term treatment of congestive heart failure in combination with digitalis and/or diuretics.
