FDA发布Dexferrum(右旋糖酐铁注射液)安全风险警告信息
    近日,美国食品药品监督管理局(FDA)和American Regent Laboratories Inc. 共同向医护人员发布Dexferrum(右旋糖酐铁注射液)说明书增加的安全风险信息,警示医护人员经非消化道使用右旋糖酐铁可能发生包含致死性的过敏反应。

    Dexferrum是American Regent Laboratories Inc.生产的缺铁性贫血治疗药物。此次,经修订的Dexferrum说明书增加[黑框警示]信息,建议患者按照治疗剂量在首次使用Dexferrum前,应按照规定剂量用药,用药期间严密观察患者的过敏反应体征或症状。但是现有资料显示,即使在耐受剂量下,也有发生致命性过敏反应的情况。

    FDA提醒:有药物过敏史的患者发生右旋糖酐铁过敏反应的风险可能增高;建议必须在具备抢救条件、可以进行过敏反应诊断和经过培训的专业人员实施的条件下使用Dexferrum。

性状: 本品为深褐色的胶体溶液。

药理毒理
抗贫血药。铁为血红蛋白及肌红蛋白的主要组成成份。血红蛋白为红细胞中主要携氧者。肌红蛋白系肌肉细胞贮存氧的部位,以助肌肉运动时供氧需要。与三羧循环有关的大多数酶均含铁,或仅在铁存在时才能发挥作用。所以对缺铁患者积极补充铁剂后,除血红蛋白合成加速外,与组织缺铁和含铁酶活性降低的有关症状如生长迟缓、行动异常、体力不足、粘膜组织变化以及皮肤指甲病变也均能逐渐得以纠正。

药代动力学
本品由于分子较大,须由淋巴管吸收再转入血液,所以注射后血浓度提高较慢,约24~48小时血药浓度达高峰。铁吸收后与转铁蛋白结合在血中循环,以供造红细胞之用,也可以铁蛋白或含铁血黄素形式累积在肝、脾、骨髓及其他网状内皮组织。铁在人体中每日排泄极微量,见于尿、粪、汗液、脱落的肠粘膜细胞及酶内,丧失总量为0.5~1.0mg。

适应症
用于防治多种原因引起的缺铁性贫血,如慢性失血、营养不良或铁吸收障碍,以及儿童、孕妇需铁量增加而食物供给不足等。

用法用量
深部肌内注射 一次50~100mg(Fe) 1~3日1次。 小儿体重超过6kg者 一次25mg(Fe) 1日1次。 小儿体重6kg以下者 一次12.5mg(Fe) 1日1次。

不良反应
本品注射后,可产生局部疼痛及色素沉着。

禁忌症
(1)对铁剂过敏者。
(2)非缺铁性贫血。
(3)肝肾功能严重损害者。
(4)胃、十二指肠溃疡患者。
(5)溃疡性结肠炎患者。
(6)血色素沉着、含铁血黄素沉着症患者。

注意事项
1.适于不能耐受口服铁剂的缺铁性贫血患者,或需迅速纠正缺铁患者。
2.注射本品后血红蛋白未见逐步升高者应即停药。
3.严重肝、肾功能不全者禁用。

孕妇及哺乳期妇女用药
尚不明确。

GENERIC NAME: IRON DEXTRAN - INJECTION (EYE-urn DEX-tran)

BRAND NAME(S): Dexferrum, Imferon, INFeD

WARNING: Infrequently, iron dextran for injection has caused severe (sometimes fatal) allergic reactions. It should be used only when clearly needed and when a patient is unable to take iron by mouth (oral forms).

Tell your doctor immediately if you experience signs of an allergic reaction such as trouble breathing, dizziness, rash, or itchy skin.

USES: This medication is used to treat "iron-poor" blood (anemia) in people who cannot take iron by mouth because of side effects or because their anemia has not been successfully treated by it. Low iron levels can occur when the body can't get enough iron from food (poor nutrition, poor absorption) or when there is a large or long-term blood loss (e.g., hemophilia, stomach bleeding). You may also need extra iron because of blood loss during kidney dialysis. Your body may need more iron if you use the drug erythropoietin to help make new red blood cells.Iron is an important part of your red blood cells and is needed to carry oxygen in the body.

HOW TO USE: This medication is usually injected by a health care professional deep into the muscle of the buttock or slowly into a vein.Before the first full dose, a smaller test dose is given slowly to check for possible allergic effects. If no reaction is seen after one hour, the full dose may be given. You will be checked carefully for reactions by a health care worker each time you are getting the iron.Iron injections may be given once daily in small doses or as directed by your doctor. Large doses may be given in a solution and injected into a vein over several hours. Some side effects such as dizziness and flushing may be stopped by giving the drug more slowly. The dose and length of treatment is based on your age, weight, condition, and response to therapy. Your doctor will order blood tests to monitor your response.

SIDE EFFECTS: Flushing, tingling of the hands/feet, shivering, or dizziness may occur. The area around the injection site may be tender, irritated, or discolored (brown). If any of these effects persist or worsen, contact your doctor or pharmacist promptly.Some people may experience a delayed reaction 1-2 days after their treatment. These side effects usually lessen within 3 to 4 days if the medication was injected into a vein or within 3 to 7 days if the medication was injected into a muscle. Tell your doctor if any of these side effects persist or worsen more than 4 to 7 days after your treatment: back/joint/muscle aches, chills, moderate to high fever, headache, nausea/vomiting.Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.Tell your doctor right away if you have any serious side effects, including: abdominal pain, fast/slow/irregular heartbeat, severe headache, blurred vision.Get medical help right away if any of these rare but very serious side effects occur: chest pain, seizures.A very serious allergic reaction to this drug may occur. Get medical help right away if you notice any of the following symptoms of a serious allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.This is not a complete list of possible side effects. If you notice other effects not listed above, contact your doctor or pharmacist.In the US -Call your doctor for medical advice about side effects. You may report side effects to FDA at 1-800-FDA-1088.In Canada - Call your doctor for medical advice about side effects. You may report side effects to Health Canada at 1-866-234-2345.

PRECAUTIONS: Before using this medication, tell your doctor or pharmacist if you are allergic to it; or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.This medication should not be used if you have certain medical conditions. Before using this medicine, consult your doctor or pharmacist if you have: low blood count not due to low iron (e.g., vitamin B12/folate deficiency), active kidney infection.Before using this medication, tell your doctor or pharmacist your medical history, especially of: severe allergic reactions, asthma, bleeding problems (e.g., hemophilia), heart disease (e.g., chest pain, heart attack, heart failure), Hodgkin's disease, autoimmune disease (e.g., rheumatoid arthritis, ankylosing spondylitis, lupus), kidney disease/dialysis, liver disease.During pregnancy, this medication should be used only when clearly needed. Discuss the risks and benefits with your doctor.This medication passes into breast milk and may have undesirable effects on a nursing infant. Consult your doctor before breast-feeding.

DRUG INTERACTIONS: Drug interactions may change how your medications work or increase your risk for serious side effects. This document does not contain all possible drug interactions. Keep a list of all the products you use (including prescription/nonprescription drugs and herbal products) and share it with your doctor and pharmacist. Do not start, stop, or change the dosage of any medicines without your doctor's approval.Some products that may interact with this drug include: beta blockers (e.g., atenolol, propranolol), "blood thinners" (e.g., heparin, warfarin), other iron products, multivitamins with minerals.This medication may interfere with certain laboratory tests (including bilirubin, calcium, and clotting times), possibly causing false test results. Make sure laboratory personnel and all your doctors know you use this drug.

OVERDOSE: Accidental overdose of iron-containing products is a leading cause of fatal poisoning in children under six. If overdose is suspected, contact your poison control center or emergency room immediately. US residents can call the US National Poison Hotline at 1-800-222-1222. Canada residents can call a provincial poison control center.

NOTES: Laboratory and/or medical tests (e.g., complete blood count, iron) should be performed periodically to monitor your progress or check for side effects. Consult your doctor for more details.It is important to maintain a well-balanced diet to ensure adequate intake of iron, vitamins, and minerals. Good sources of iron include meats (especially liver), eggs, raisins, figs, broccoli, Brussels sprouts, beans, lentils, and iron-fortified or enriched cereals. Follow any diet recommendations given to you by your doctor.

MISSED DOSE: For the best possible benefit, it is important to receive each scheduled dose of this medication as directed. If you miss a dose, contact your doctor or pharmacist immediately to establish a new dosing schedule.

STORAGE: Not applicable. This medication is given in a clinic and will not be stored at home.
