英文药名:Desmopressin 

其他名称:AdiuretinSD、DAVRitter、DDAVP、Desurin、Desmopray、Desmo-pressin Acetate、Octosim、1-脱氨-8-D-精氨酸-加压素、去氨精氨酸加压素、醋酸去氨加压素、的斯加压素、弥凝(Minirin)、依他停。 

药理作用:去氨加压素是在加压素V2受体高亲和力同系物的研究中开发出来的,其化学结构与人体自然产生的激素精氨酸加压素相类似,但因有两处改变,故显著增强了抗利尿作用,而对平滑肌的作用却很弱,因此避免了引起升高血压的副作用。另外,使用本品高剂量,即按0.3μg/kg静脉或皮下注射,可增加血浆内促凝血因子Ⅷ的活性2-4倍,也可增加血中血管性血友病抗原因子(vWF:Ag),与此同时释放出纤维蛋白溶酶原激活质(t-PA),故可用于控制或预防某些疾病在小手术时的出血或药物诱发的出血。
本品按0.3μg/kg剂量注射后,平均值约为600pg/ml的最高血浆浓度约在1小时出现。半衰期约3-4小时。对多数患者口服或注射本品,其抗利尿作用可维持8-12小时,凝血效应大约亦维持8-12小时。 

临床应用:
临床用于:
(1)中枢性尿崩症及颅外伤或手术所致的暂时性尿崩症:用本品后可减少尿排出,增加尿渗透性,减低血浆渗透压,减少尿频和夜尿。本品一般对肾原性尿崩症无效。
(2)治疗5岁以上患有夜间遗尿症的患者。
(3)肾尿液浓缩功能试验:有助于对肾功能的鉴别,对于诊断不同部位的尿道感染尤其有效。
(4)对于轻度血友病瑞龇及Ⅰ型血友病患者,在进行小型外科手术时可控制出血或预防出血。
(5)对于因尿毒症,肝硬化以及先天的或药物诱发的血小板机能障碍而引起的出血时间过长和不明原因的出血,用本品可使出血时间缩短或恢复正常。 

禁忌症:习惯性或精神性烦渴症、不稳定性心绞痛、心功能不全、ⅡB型血管性血友病、对防腐剂过敏患者等禁用。 

不良反应 
(1)少部分病人出现头痛、恶心、胃痛、变态反应、水潴留及低钠血症。
(2)高剂量时可引起短暂的血压降低、反射性心跳快速及面部潮红、眩晕、疲乏等。
(3)注射给药时,可致注射部位疼痛、肿胀。 

制剂规格:(醋酸盐)片剂0.1mg,0.2mg;注射剂lml:41μg;鼻喷雾剂2.5ml:0.1mg(10μg/喷);滴鼻剂2.5ml:0.25mg。 

注意事项
(1)一些可释放抗力尿激素的药物,如三环类抗抑郁药、氯丙嗪、卡马西平等,可增加抗利尿作用并有引起体液潴留的危险。
(2)对婴幼儿及老年人、体液或电解质平衡紊乱、易产生颅内压增高的患者以及孕妇应谨慎使用本品,防止体液蓄积。
(3)1岁以下婴儿必须在医院监护下实行肾浓缩功能试验。
(4)用药期间需要监测患者的尿量、渗透压和体重,对有些病例还需测试血浆渗透压。
(5)用于止血,对需要服用利尿剂的病人,必需采取适当的措施,防止体液积蓄过多。
(6)在治疗遗尿症时,用药前1小时至用药后8小时内需限制饮水量。当用于诊断检查时,用药前1小时至用药后8小时内饮水量不得超过500ml。
(7)超量给药会增加水潴留和低钠血症的危险,治疗低钠血症时的用药应视具体病情而定。对无症状的低钠血症病人,除停用去氨加压素外,还应限制饮水量。对有症状的病人,可根据症状输入等渗或高渗氯化钠液,当体液潴留症状严重时(抽搐或神志不清),需加服呋塞米。
(8)鼻腔用药后,鼻粘膜若出现瘢:水肿或其他病变时,应停用鼻腔给药法。
(9)吲哚美辛会加重患者对本品的反应,但不会影响其反应持续时间。 

GENERIC NAME: DESMOPRESSIN 1.5 MG/ML SPRAY - NASAL (dess-mow-PRESS-in)

BRAND NAME(S): Stimate
 
USES: Desmopressin is used in people with certain bleeding problems (hemophilia A, type 1 von Willebrand's disease) to control bleeding caused by surgery, injuries, or heavy menstrual periods. This medication is a man-made form of a certain substance in the body called vasopressin that helps with normal blood clotting.

HOW TO USE: Read the patient instructions that come with this product before you start using desmopressin and each time you get a refill. If you have any questions, consult your doctor or pharmacist.Before using, follow the instructions on how to properly prime the bottle. Spray this medication in the nose as directed by your doctor. If the dose is more than 1 spray, spray half the dose in each nostril. For example, if the dose is 2 sprays, give 1 spray in each nostril.Keep track of the number of sprays used from the container. Discard the container after you have used the number of sprays specified on the package.If this medication is used by a child, a parent or other responsible adult should supervise and help the child use this medication properly.Limit drinking of water and other fluids while using this medication, especially in children and older adults. Consult your doctor for details. If you find that you are drinking more fluids than directed, tell your doctor immediately. Your treatment will need to be adjusted. (See also Side Effects and Precautions sections.)Dosage is based on your medical condition and response to treatment. You may need to use another dose if your doctor directs you to do so. This medication is not used on a regular schedule. Do not use more desmopressin or use it more often or for longer than prescribed.If using before surgery, give the dose 2 hours before the procedure as directed.Tell your doctor if your bleeding is not controlled.

SIDE EFFECTS: Headache, runny/stuffy nose, nausea, upset stomach, or flushing of the face may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.Desmopressin reduces the amount of urine your kidneys make. It may affect the balance of water and minerals in your body. Desmopressin can infrequently cause a low level of sodium in the blood, which can be serious and possibly life-threatening. Drinking too much water or other fluids increases the risk of having a low level of sodium in the blood. Therefore, follow your doctor's directions for limiting fluids. Seek immediate medical attention if you have any of these unlikely but serious symptoms of a low level of sodium in the blood: loss of appetite, severe nausea, vomiting, severe headache, muscle weakness/spasms/cramps, sudden weight gain, unusual tiredness, unusual severe drowsiness, mental/mood changes (such as confusion, hallucinations, irritability), loss of consciousness, seizures, slow/shallow breathing.A very serious allergic reaction to this drug is rare. However, seek immediate medical attention if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.This is not a complete list of possible side effects. If you notice other effects not listed above, contact your doctor or pharmacist.In the US -Call your doctor for medical advice about side effects. You may report side effects to FDA at 1-800-FDA-1088.In Canada - Call your doctor for medical advice about side effects. You may report side effects to Health Canada at 1-866-234-2345.

PRECAUTIONS: Before using desmopressin, tell your doctor or pharmacist if you are allergic to it; or if you have any other allergies.This medication should not be used if you have certain medical conditions. Before using this medicine, consult your doctor or pharmacist if you have: kidney disease, a low level of sodium in the blood (current or past hyponatremia).Before using this medication, tell your doctor or pharmacist your medical history, especially of: high blood pressure, heart disease (such as blocked blood vessels in the heart), conditions that may increase your risk of fluid/mineral imbalance (such as cystic fibrosis, heart failure), the urge to drink too much water without being thirsty.If you have lost too much body water (become dehydrated), your doctor will correct that condition first before starting treatment with desmopressin.Limit alcohol because it can interfere with how well desmopressin works.Changes inside the nose (such as scarring, swelling, congestion, or blockage) may affect how well desmopressin nasal spray is absorbed. Tell your doctor if you have any of these changes inside the nose because he or she may need to adjust your treatment.Tell the doctor immediately if you develop any illness that can cause water/mineral imbalance (including fever, diarrhea, vomiting, infections such as the flu) or if you experience conditions that require drinking more fluids (for example, exposure to very hot weather, strenuous exercise, persistent sweating). The doctor may need to stop or adjust desmopressin treatment, especially in children and older adults.Children may be more sensitive to the side effects of this drug, especially water/mineral imbalance and a low level of sodium in the blood.Kidney function declines as you grow older. This medication is removed by the kidneys. Therefore, older adults may be at greater risk for water/mineral imbalance and a low level of sodium in the blood while using this drug.During pregnancy, this medication should be used only when clearly needed. Discuss the risks and benefits with your doctor.It is not known whether this drug passes into breast milk. Consult your doctor before breast-feeding.

DRUG INTERACTIONS: Your doctor or pharmacist may already be aware of any possible drug interactions and may be monitoring you for them. Do not start, stop, or change the dosage of any medicine before checking with your doctor or pharmacist first.Before using this medication, tell your doctor or pharmacist of all prescription and nonprescription/herbal products you may use, especially of: lithium.Other medications besides desmopressin may also affect the balance of water and sodium in your body. Therefore, tell your doctor or pharmacist about all the products you take, including: narcotic pain relievers (such as morphine), nonsteroidal anti-inflammatory drugs (NSAIDs such as ibuprofen, naproxen), psychiatric medicines (such as chlorpromazine), drugs to treat seizures (such as carbamazepine), SSRI antidepressants (such as sertraline), tricyclic antidepressants (such as amitriptyline), "water pills" (diuretics such as furosemide).This document does not contain all possible interactions. Therefore, before using this product, tell your doctor or pharmacist of all the products you use. Keep a list of all your medications with you, and share the list with your doctor and pharmacist.

OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. US residents can call the US National Poison Hotline at 1-800-222-1222. Canada residents can call a provincial poison control center. Symptoms of overdose may include: confusion, drowsiness, persistent/severe headache, sudden weight gain.

NOTES: Do not share this medication with others.Laboratory and/or medical tests (such as blood tests) may be performed periodically to monitor your progress or check for side effects. Consult your doctor for more details.

MISSED DOSE: This medication is used as needed for bleeding episodes as directed by your doctor. It is not for regular (scheduled) use.

STORAGE: Store the US product in the refrigerator between 36-46 degrees F (2-8 degrees C). Do not freeze. When traveling, the US product may remain at room temperature at 72 degrees F (22 degrees C) away from light and moisture for up to 3 weeks. Discard after 3 weeks out of the refrigerator.Store the Canadian product in the refrigerator between 36-46 degrees F (2-8 degrees C). Do not freeze.Keep the bottle upright. Do not store in the bathroom. Keep all medicines away from children and pets.Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed.
