分类名称 一级分类:抗生素 二级分类:其他β内酰胺类 三级分类: 药品英文名 Ertapenem 药品别名 艾他培南、Etapenem、Invanz、MK-0826、L-749、345 药物剂型 厄他培南注射剂:1.0g/支。室温保存。 药理作用 本品是一种新型碳青霉烯类抗生素,通过与青霉素结合蛋白(PBP)结合,干扰细菌细胞壁的合成,导致细菌生长繁殖受抑制,少数出现细胞溶解。本品对甲氧西林敏感金葡菌、肺炎链球菌、化脓性链球菌等革兰阳性菌、肠杆菌科细菌具有高度抗菌活性;嗜血杆菌属、卡他莫拉菌、脑膜炎奈瑟球菌等对本品高度敏感,但对甲氧西林耐药葡萄球菌、肠球菌属、铜绿假单胞菌、不动杆菌属等细菌对本品耐药。本品对大多数青霉素酶、头孢菌素酶和超广谱β-内酰胺酶稳定,但可被金属酶水解。本品对人类肾脱氢肽酶-Ⅰ稳定,不需与西司他丁等联合应用。 药动学 静滴本品0.5g、1g和2g后30min内达血药浓度峰值(Cmax),分别为71.3mg/L、137.0mg/L和255.9mg/L。肌注本品1g后生物利用度约为90%,达峰时间为2.3h,Cmax为67mg/L。本品的血浆蛋白结合率为92%~95%,浓度低时蛋白结合率较高。主要经肾脏排出,其血浆半衰期为4.3~4.6h。健康青年志愿者静脉应用同位素标记的本品1g后,尿液和胆汁中药物排除率分别为80%和10%,尿液中药物原形和代谢产物各占40%。
适应证 本品适用于敏感菌所致的下列中、重度感染:复杂性腹腔感染、复杂性皮肤软组织感染、社区获得性肺炎、复杂性尿路感染、急性盆腔感染、严重肠杆菌科细菌感染等。 禁忌证 1.对本品或其他碳青霉烯类过敏者禁用 2.本品肌注由利多卡因溶液稀释,不得改用于静脉给药,也不得用于利多卡因过敏者或合并严重休克、房室传导阻滞等其他利多卡因禁忌证患者。 注意事项 1.在孕妇中尚缺乏足够的研究,因此不推荐使用。 2.本品经乳汁分泌,哺乳期妇女应用本品应停止哺乳。 3.尚缺乏本品用于儿童感染的疗效和安全性的资料,不推荐将本品应用于18岁以下患者。 4.老年人应根据肾功能调整剂量。 5.本品不能溶解于葡萄糖溶液中,也不宜与其他药物混合。 6.溶解后立即使用,存放时间不能超过24h。 不良反应 腹泻、恶心、呕吐等胃肠道反应,静脉炎,头痛以及女性阴道炎等。丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)和肌酐等升高。应用本品后患者癫痫发生率为0.5%。 用法用量 成人:每次1g,每日1次,静脉或肌注给药。静脉滴注时间应大于30min,肌注可用于静脉用药的序贯治疗。 肌酐清除率≤30ml/min者剂量调整为每次0.5g,每日1次;如在给药后6h内血液透析,透析后给予补充剂量0.15g。 本品静脉滴注时每1g应溶解于50ml以上生理盐水中,每次静滴时间应大于30min;供肌注时每1g应溶解于1%利多卡因溶液做深部肌内注射。 药物相应作用 丙磺舒可延长本品血清半衰期,提高其血药浓度。 Invanz Company: Merck Approval Status: Approved November 2001 Treatment for: Bacterial infections Areas: Dermatology/Plastic Surgery; Nephrology/Urology; Obstetrics/Gynecology; Pulmonary/Respiratory Diseases
General Information Invanz is a once-daily injectable antibiotic approved for use in adults for the treatment of moderate to severe infections caused by common gram-positive and gram-negative aerobic and anaerobic bacteria. Included, are community-acquired pneumonia, intra-abdominal, skin, urinary tract, kidney and post-surgical gynecological infections.
Prior to administration, Invanz must be reconstituted and diluted.
Clinical Results The approval of Invanz is supported by studies conducted in more than 1,900 subjects, in five infectious disease categories. Treatment ranged from three to 14 days. Invanz exhibited an excellent safety profile and showed success rates equivalent to those of comparator antibiotics.
Side Effects Adverse events associated with the use of Invanz may include (but are not limited to) the following: Diarrhea Infused vein complications Nausea Headache Vaginitis in females
Mechanism of Action Invanz (ertapenem sodium) is a structurally unique 1-(beta) methyl-carbapenem related to a class of antibiotics known as beta-lactams, which includes penicillins and cephalosporins. Invanz works by blocking the formation of bacterial cell walls, thereby causing cell death.
