介绍
目前治疗青光眼的主要方法是局部使用降眼压药物,复合制剂指把2种或2种以上的药物放在一个滴眼瓶里,它增强了降眼压作用,减少了副反应,能提高患者的依从性。目前,已在临床上使用的复合制剂主要有胆碱能激动剂与肾上腺索能激动剂复合制剂、β-受体阻滞剂与胆碱能激动剂复合制剂、局部碳酸酐酶抑制剂β-受体阻滞剂复合制剂、前列腺素类药物β-受体阻滞剂复合制剂和α2受体激动剂与β-受体阻滞剂复合制剂。大多数复合制剂都含有β-受体阻滞剂,随着前列腺索类药物逐渐取代β-受体阻滞剂成为一线用药,含前列腺素类药物的复合制剂可能成为未来的发展趋势和青光眼治疗的新策略。尽管如此,复合制剂也存在自身的缺点,并非适用于所有的患者。因此,临床医生应该根据个体化用药原则,为患者制定出合理的用药方案。
眼压是眼球内容物作用于眼球壁的压力。眼压如果超过了自身视神经的承受能力,则会造成视神经损伤。因此,降低眼压是治疗青光眼的主要目的和方法。近年来,激光和手术等治疗方法发展迅速,但是局部用降眼压药物仍是治疗青光眼的主要选择。青光眼早期治疗,首先选用一种药物,通常是β-受体阻滞剂,但现在许多发达国家,前列腺素类药物已逐渐取代β-受体阻滞剂而成为一线用药。此外,随着病情的进展,很多病人仅用一种药物不能够将眼压控制到靶眼压水平,部分患者在早期就需要用2种或2种以上的药物。

复合制剂的定义与特点
青光眼联合用药是指在一种药物不能控制眼压的情况下,加用另外一种或几种降眼压药物。联合用药分为非固定配方(unfixed combination)和固定配方 (fixed combination)两种。前者指把同时使用的药物放在各自的滴眼瓶里,后者指把2种或2种以上的药物放在一个滴眼瓶里, 即复合制剂(fixed combination )。青光眼药物的复合制剂出现于20世纪60年代早期。其主要的优点有:
(1)两种药物的协同作用增强了降眼压效果。
(2)减少了单种药物的剂量和用药次数,而且减少了每天滴药的总滴数及可能的副反应。
(3)取消了2种药物滴眼之间的等待时间。
(4)可减少防腐剂(如苯扎氯铵)引起的上皮细胞毒性。
(5)减轻了患者的经济负担。
(6)提高病人的耐受程度和依从性。
(7)改善最终接受滤过性手术的病人的预后。
复合制剂的主要缺点在于:所含药物的最佳滴眼次数可能不同,这样就不能使两者同时发挥最大的效果。可能出现一种成分过量, 副反应增加,或者一种成分剂量不足,达不到理想降眼压效果。而且复合制剂的固定浓度也限制了其应用,比如有的病人对0.25%的噻吗心安反应最好,但是所有复合制剂噻吗心安的浓度均为0.5%。在这种情况下,应建议病人选择非同定配方联合用药。因此,临床医生应该根据个体化用药原则,为每个病人制定出合理的用药方案。

药品简介
曲伏前列素与噻吗心安复合制剂(travoprost/timolol fixed combination):商品名为DuoTrav (Alton公司),为0.004%曲伏前列素和0.5%噻吗心安的复合制剂,2005年上市。该复合制剂可以有效地降低眼压,且早晚用药的疗效相似旧。Barnebey等将263例POAG或OblT随机分组治疗3个月,发现复合制剂组眼压下降平均值比噻吗心安组大1.9~3.3mmHg,比曲伏前列素组大0.9~2.4 mmHg。且复合制剂的副反应用与单用两种药物无明显差别。Schuman等比较了该制剂与非固定配方联合使用这两种药物的降压作用,结果显示两种方法眼压下降幅度相似。

【組成】
每一毫升溶液含Travoprost 40 mcg/ml及Timolol 5 mg/ml (相當於Timolol maleate 6.8 mg/ml)
 
【適應症】
對乙型阻斷劑或前列腺素類似物眼用製劑治療反應不佳的隅角開放性青光眼或高眼壓患者,用以降低其眼內壓。
 
【用法用量與投與方式】
本藥須由醫師處方使用
用於成人(包括老年人)
每天早上或晚上點一滴於眼睛,每日1次,請於每日同一時間使用。建議使用後輕輕閉上眼睛,並用手指壓住鼻側之眼角,可降低點眼液的全身性吸收,及減少全身性副作用發生。
若使用一種以上局部眼用藥,應間隔至少5分鐘以上【參見與其他藥品的交互作用】。
如忘記點藥,請於下一次應點藥的時間再點藥即可。
每日用量請勿超過1滴。若以DUOTRAV點眼液取代其他抗青光眼眼用製劑,應先停用該藥,並於次日開始使用DUOTRAV 點眼液。
用於兒童:
DUOTRAV點眼液用於18歲以下患者的效果及安全性尚未確立,在無進一步相關資料前不建議使用。
用於肝及腎功能不全者:未進行DUOTRAV點眼液,或timolol 5 mg/ml點眼液用於肝或腎功能不全者的研究。
已有travoprost用於輕度至重度肝功能不全者,或輕度至重度腎功能不全者(肌酸酐廓清率低於14 ml/min)的研究。這類患者不需調整劑量。
本藥為眼用製劑。
首次使用前,請將封口膠膜打開後立即使用。請小心不要使眼瞼、周圍區域或其他表面碰觸到滴瓶口,以避免污染滴瓶口及藥水。

【禁忌】
對travoprost, timolol或任一賦形劑過敏。
支氣管氣喘或曾有支氣管氣喘或嚴重慢性阻塞性肺病病史。
竇心跳徐緩、二或三級房室阻斷、心衰竭或心源性休克。
重度過敏性鼻炎及支氣管過度敏感、角膜萎縮、對其他乙型阻斷劑過敏。
 
【特殊警告及注意事項】
全身性作用
如同其他局部眼用製劑,travoprost和timolol會被吸收到全身。由於timolol對乙型腎上腺素接受體的影響,因此可能發生與全身性乙型腎上腺素阻斷劑同類型的心血管及肺部副作用。在開始使用timolol前,應先使心衰竭獲得適當控制。有嚴重心臟疾病病史的患者應注意心衰竭的徵兆,並測定其脈搏。曾有服用timolol maleate發生呼吸反應及心臟反應的報告,包括氣喘病人死於氣管痙癴和罕見的與心衰竭有關的死亡。乙型腎上腺素阻斷劑用於自發性低血糖患者或糖尿病患者(尤其是不穩定的糖尿病患者)應謹慎使用,因為這類藥物會掩蓋急性低血糖徵兆及症狀。它們也可能會掩蓋甲狀腺機能亢進的徵兆,並使Prizmental心絞痛、嚴重週邊及中樞循環疾病及低血壓惡化。
過敏反應
服用乙型腎上腺素阻斷劑時,具異位性病史或對各種過敏原有嚴重過敏反應病史的患者,可能對治療過敏反應所使用的一般劑量的腎上腺素沒有反應。

How does DuoTrav work? What will it do for me?
This product combination consists of two different medications: travoprost and timolol. Travoprost belongs to a class of medications called prostaglandin analogues. Timolol belongs to a class of medications called beta-adrenergic receptor blockers (also known as beta-blockers).

Travoprost - timolol is used to reduce the pressure inside the eye for people with open-angle glaucoma or ocular hypertension (increased pressure in the eye in people who do not have a complete response to treatment with only one medication). This medication is used when your doctor feels it is appropriate to use both travoprost and timolol. This medication is not to be used as initial treatment.

Fluid is constantly being formed and drained out of the eye. When this fluid does not drain out of the eye properly, pressure inside the eye increases. Travoprost - timolol works by reducing the amount of fluid produced by the eye and by increasing the flow of fluid out of the eye.

Your doctor may have suggested this medication for conditions other than those listed in these drug information articles. As well, some forms of this medication may not be used for all of the conditions discussed here. If you have not discussed this with your doctor or are not sure why you are being given this medication, speak to your doctor. Do not stop using this medication without consulting your doctor.

Do not give this medication to anyone else, even if they have the same symptoms as you do. It can be harmful for people to use this medication if their doctor has not prescribed it.

How should I use DuoTrav?
The recommended adult dose of travoprost - timolol is one drop in the affected eye(s) once daily each morning. Do not use more than once a day.

Many things can affect the dose of a medication that a person needs, such as body weight, other medical conditions, and other medications. If your doctor has recommended a dose different from the ones listed here, do not change the way that you are using the medication without consulting your doctor.

To use the eye drops:
Wash your hands before using the eye drops.
Remove the cap and place it in a clean location. To avoid possible contamination, keep the tip of the container away from contact with any surface.
Tilt the head back and look towards the ceiling.
With your index finger, gently pull the lower eyelid down and away from the eye to form a pouch.
Apply one drop into the pouch but do not allow the tip of the container to touch the eye or areas around the eye.
Gently apply pressure to the inner corner of the eye (at the bridge of the nose) for about 30 seconds (this is called nasolacrimal occlusion). This prevents the medication from dripping down through the tear duct and entering the bloodstream, which could cause you to experience some side effects.
Repeat with the other eye, if prescribed by your physician.
Wash your hands again to remove any medication.
Do not allow the dropper tip of the bottle to touch the eye or other surrounding structures. This can contaminate the tip with common bacteria known to cause eye infections. Serious damage to the eye, possibly leading to loss of vision may result if you use eye drop solutions that have become contaminated.

If you use other eye drops as well, wait at least 5 minutes before using them.

If you wear contact lenses, remove them before instilling travoprost - timolol into your eye. You may put your contact lenses back in 15 minutes after using the medication.

It is important to use this medication exactly as prescribed by your doctor. If you miss a dose, skip the missed dose and continue with your regular dosing schedule. Do not administer a double dose to make up for a missed one. If you are not sure what to do after missing a dose, contact your doctor or pharmacist for advice.

Store this medication at room temperature and keep it out of the reach of children.

Do not dispose of medications in wastewater (e.g. down the sink or in the toilet) or in household garbage. Ask your pharmacist how to dispose of medications that are no longer needed or have expired.

What form(s) does DuoTrav come in?
Each mL of sterile, isotonic, buffered, preserved, aqueous solution contains travoprost 0.04 mg and timolol 5.0 mg. Nonmedicinal ingredients: benzalkonium chloride, boric acid, edetate disodium, hydrochloric acid, mannitol, polyoxyl 40 hydrogenated castor oil, purified water, and tromethamine.

Some medications may have other generic brands available. Always ask your doctor or pharmacist about the safety of switching between brands of the same medication.

Who should NOT take DuoTrav?
Travoprost - timolol should not be used by anyone who:
is allergic to travoprost, timolol, or to any of the ingredients of the medication
has asthma or has had asthma in the past
has certain heart diseases or conditions
cardiogenic shock (heart cannot provide enough oxygen to the body)
second-degree or third-degree atrioventricular (AV) block (electrical problems)
sinus bradycardia (slow heartbeat)
uncontrolled heart failure (heart cannot pump effectively)
has severe chronic obstructive pulmonary disease (COPD)
