    先灵葆雅公司(Schering-Plough)宣布,FDA已批准其氯雷他定+伪麻黄碱复方制剂(loratadine+pseudoephedrine,Claritin-D缓释制剂)暂时缓解普通感冒引发的鼻充血症状,本品可用于成人、12岁和12岁以上的青少年患者。FDA此前曾批准本品暂时缓解季节性变应性鼻炎和其它上呼吸道变态反应导致的鼻流涕等相关症状。
   
    抗组胺药氯雷他定可缓解打喷嚏、鼻分泌物过多、鼻部瘙痒等症状,伪麻黄碱可收缩血管,减少鼻充血部位的血流,改善呼吸气流。本品有Claritin-D 12h和Claritin-D 24h 2种规格,其疗效分别可持续12小时和24小时。本品1~3小时即可起效,常见的不良反应有眩晕、头痛、失眠、恶心、腹泻、腹部不适、口干、皮肤瘙痒、尿液变色等。

    下列药物与本品有相互作用,不宜同时服用:
    单胺氧化酶抑制剂(MAOI),如异卡波肼(isocarboxazid,Marplan)、苯乙肼(phenelzine,Nardil)、反苯环丙胺(tranylcypromine,Parnate)等;
    心血管药物,如:普奈洛尔(propranolol,Inderal)、美托洛尔(metoprolol,Lopressor)、地高辛(digoxin,Lanoxin)、利血平(reserpine ,Serpasil)、甲基多巴 (methyldopa,Aldomet)等;
    咖啡因(caffeine)、苯丙胺(amphetamines)、减充血剂和减肥药。

Claritin-D
Generic Name: loratadine and pseudoephedrine (lor AT a deen and SOO doe ee FED rin)
Brand Names: Alavert D-12, Claritin-D, Claritin-D 24 Hour

What is Claritin-D?
Claritin-D contains a combination of loratadine and pseudoephedrine. Loratadine is an antihistamine that reduces the natural chemical histamine in the body. Histamine can produce symptoms of sneezing, itching, watery eyes, and runny nose.

Pseudoephedrine in Claritin-D is a decongestant that shrinks blood vessels in the nasal passages. Dilated blood vessels can cause nasal congestion (stuffy nose).

Claritin-D is used to treat sneezing, runny or stuffy nose, itchy or watery eyes, hives, skin rash, itching, and other symptoms of allergies and the common cold.

Claritin-D may also be used for other purposes not listed in this medication guide.

Important information about Claritin-D.
Do not give Claritin-D to a child younger than 2 years old. Always ask a doctor before giving a cough or cold medicine to a child. Death can occur from the misuse of cough and cold medicines in very young children. Do not use Claritin-D if you have taken an MAO inhibitor such as furazolidone (Furoxone), isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam), or tranylcypromine (Parnate) in the last 14 days. Serious, life threatening side effects can occur if you use Claritin-D before the MAO inhibitor has cleared from your body.

Ask a doctor or pharmacist before using any other cold, allergy, or sleep medicine. Antihistamines and decongestants are contained in many combination medicines. Taking certain products together can cause you to get too much of a certain drug. Check the label to see if a medicine contains an antihistamine or decongestant. Claritin-D may cause blurred vision or impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert and able to see clearly. Drinking alcohol can increase certain side effects of Claritin-D.

Before taking Claritin-D.
Do not use Claritin-D if you have taken an MAO inhibitor such as furazolidone (Furoxone), isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam), or tranylcypromine (Parnate) in the last 14 days. Serious, life threatening side effects can occur if you use Claritin-D before the MAO inhibitor has cleared from your body.

Ask a doctor or pharmacist if it is safe for you to take Claritin-D if you have:
kidney disease;
glaucoma;
heart disease or high blood pressure;
diabetes;
thyroid disorder;
an enlarged prostate; or
problems with urination.

FDA pregnancy category B. Claritin-D is not expected to harm an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment with Claritin-D. It is not known whether loratadine and pseudoephedrine passes into breast milk or if it could harm a nursing baby. Do not use Claritin-D without telling your doctor if you are breast-feeding a baby.

How should I take Claritin-D?
Use Claritin-D exactly as directed on the label, or as prescribed by your doctor. Do not use in larger or smaller amounts or for longer than recommended. Cold or allergy medicine is usually taken only for a short time until your symptoms clear up.

Do not give Claritin-D to a child younger than 2 years old. Always ask a doctor before giving a cough or cold medicine to a child. Death can occur from the misuse of cough and cold medicines in very young children. Do not crush, chew, or break an extended-release Claritin-D 24 hours tablet. Swallow it whole. Breaking the pill may cause too much of the drug to be released at one time. Talk with your doctor if your symptoms do not improve after 7 days of treatment, or if you have a fever with a headache, cough, or skin rash.

If you need to have surgery, tell the surgeon ahead of time if you have taken a cold or allergy medicine within the past few days.

Claritin-D can cause unusual results with allergy skin tests. Tell any doctor who treats you that you are taking an antihistamine.

Store Claritin-D at room temperature away from moisture and heat.

What happens if I miss a dose?
Since cold or allergy medicine is taken as needed, you may not be on a dosing schedule. If you are taking the medication regularly, take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.

What happens if I overdose?
Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

Overdose symptoms may include feeling restless or nervous, nausea, vomiting, stomach pain, dizziness, drowsiness, dry mouth, warmth or tingly feeling, or seizure (convulsions).

What should I avoid while taking Claritin-D?
Claritin-D may cause blurred vision or impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert and able to see clearly. Drinking alcohol can increase certain side effects of Claritin-D.

Avoid taking diet pills, caffeine pills, or other stimulants (such as ADHD medications) without your doctor's advice. Taking a stimulant together with a decongestant can increase your risk of unpleasant side effects.

Ask a doctor or pharmacist before using any other cold, allergy, or sleep medicine. Antihistamines and decongestants are contained in many combination medicines. Taking certain products together can cause you to get too much of a certain drug. Check the label to see if a medicine contains an antihistamine or decongestant.

Claritin-D side effects
Get emergency medical help if you have any of these signs of an allergic reaction to Claritin-D: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Stop using Claritin-D and call your doctor at once if you have a serious side effect such as:
fast, pounding, or uneven heartbeat;
confusion, hallucinations, unusual thoughts or behavior;
severe dizziness, anxiety, restless feeling, or nervousness;
dangerously high blood pressure (severe headache, blurred vision, buzzing in your ears, chest pain, shortness of breath, uneven heartbeats, seizure);
easy bruising or bleeding, unusual weakness, fever, chills, body aches, flu symptoms; or
urinating less than usual or not at all.

Less serious Claritin-D side effects may include:
blurred vision;
dry mouth;
nausea, stomach pain, constipation;
mild loss of appetite, stomach upset;
warmth, redness, or tingly feeling under your skin;
sleep problems (insomnia);
restless or excitability (especially in children);
skin rash or itching;
dizziness, drowsiness;
problems with memory or concentration; or
mild ringing in your ears.

This is not a complete list of side effects and others may occur. Call your doctor for medical advice about side effects. You may report side effects to FDA at 1-800-FDA-1088.

What other drugs will affect Claritin-D?
Cold or allergy medicine, sedatives, narcotic pain medicine, sleeping pills, muscle relaxers, and medicine for seizures, depression or anxiety can add to sleepiness caused by loratadine. Tell your doctor if you regularly use any of these medicines, or any other cold or allergy medicine.

Tell your doctor about all other medicines you use, especially:
blood pressure medications;
a diuretic (water pill);
medication to treat irritable bowel syndrome;
bladder or urinary medications such as oxybutynin (Ditropan, Oxytrol) or tolterodine (Detrol);
aspirin or salicylates (such as Disalcid, Doan's Pills, Dolobid, Salflex, Tricosal, and others);
a beta-blocker such as atenolol (Tenormin, Tenoretic), carvedilol (Coreg), labetalol (Normodyne, Trandate), metoprolol (Lopressor, Toprol), nadolol (Corgard), propranolol (Inderal, InnoPran), sotalol (Betapace), and others; or
an antidepressant such as amitriptyline (Elavil, Vanatrip), doxepin (Sinequan), desipramine (Norpramin), imipramine (Janimine, Tofranil), nortriptyline (Pamelor), and others.

This list is not complete and other drugs may interact with Claritin-D. Tell your doctor about all medications you use. This includes prescription, over-the-counter, vitamin, and herbal products. Do not start a new medication without telling your doctor.
